The length you order will be the total length from tip-to-tip. I do add just a bit of extra slack in order to allow the bracelet to bend around the wrist easily. The best way to determine the size you need is to wrap a piece of string around your wrist where you'd like it to fit, and then measure the string. If you're between sizes, I'd recommend to order the next size up, which will give you a nice relaxed fit and some extra wiggle room.
